Students planning required degree recitals (e.g. Junior, Senior or Graduate Recitals) must select three faculty committee members to evaluate their recitals. The committees must consist of:
- The student's applied instructor
- An additional faculty member in the student's applied area
- Any faculty member
This form serves a confirmation from your faculty panel for your recital. You must submit this completed form (with all three confirmations) before your performance can be authorized and scheduled on the School of Music events calendar or before rehearsals in the PERF RH can be scheduled. Please fill out the required information including JPEG, JPG, PDF, or PNG attachments of Faculty confirmation (such as an email, written note, signature, etc.).
